Our Lady appeared in the year 1830 in a vision to Saint Catherine Laboure, and instructed her to have a two-sided medal struck after the model of the vision. This image is the reverse side of that model.
The twelve stars signify the Apostles—the Church surrounding Mary—as well as her crown of Rev 12:1. The bar beneath the cross signifies the world and the foot of the Cross. The ‘M’ interleaving the bar represents Mary’s position at the foot of the Cross, and her integral role in her Son’s redemptive work. And the two hearts represent the loving, sorrowful hearts of Jesus and Mary.
